Will my insurance cover the therapy visit?
Before your first appointment, we recommend being proactive and verify your child’s benefit coverage through your insurance provider.
Most insurance companies will reimburse for the initial speech and language evaluation. However, insurance benefits are plan-specific, therefore your plan may not be inclusive and does not guarantee coverage for therapy. Contact your insurance carrier to determine whether your child's therapy session will be covered.
We try to make your first visit with us a breeze. We have conveniently made available our forms for you to complete at your leisure.
Please download, print and complete the forms at your convenience.
Oh! And don't forget to bring them on your initial visit.

Initial visit please bring :
- Client Intake Form
- Government Issued Identification (i.e. Driver License, Passport)
- Doctors Referral (may be faxed prior)
- Childs' Social Security card
- Insurance card
